Mathurs LLC is not an insurance carrier, broker, agent, law firm, or compliance certifier.
You remain solely responsible for verifying insurance coverage adequacy and compliance.
3. AI & Automated Extraction Disclaimer
(“Human-in-the-Loop” Requirement)
The platform utilizes third-party Artificial Intelligence (AI) and Large Language Models (LLMs), including but not limited to:
Microsoft Azure
Google Gemini
Probabilistic Nature of AI
You acknowledge that:
AI systems are probabilistic and non-deterministic
Outputs may contain inaccuracies, omissions, or “hallucinations”
Endorsements, limits, dates, and ratings may be misinterpreted
Data may be extracted incorrectly or incompletely
Mandatory Human Verification
All AI-extracted data must be manually reviewed and confirmed by a qualified human user before being relied upon for:
Compliance determinations
Insurance verification
Contractual decisions
Regulatory reporting
Business operations
Failure to independently verify extracted data is solely your responsibility.
No Warranty of Accuracy
Mathurs LLC makes no representations or warranties regarding:
Accuracy of extracted data
Detection of endorsements
Identification of missing coverages
Carrier ratings
Expiration dates
Compliance determinations
The Services are a productivity aid only.
4. Compliance Status & Notification Disclaimer
Informational Indicators Only
Any “Compliant,” “Non-Compliant,” or similar dashboard status is:
Generated based on user-defined templates
Dependent on uploaded data
Informational only
Not a legal determination
Mathurs LLC does not guarantee that:
All deficiencies will be detected
All compliance gaps will be flagged
A compliant status ensures legal compliance
Alert & Notification Limitations
Automated alerts (email, SMS, dashboard notifications) may fail due to:
Server outages
Infrastructure interruptions
Third-party email provider issues
Spam filters
Data processing delays
AI extraction errors
You are responsible for:
Maintaining independent tracking systems
Whitelisting @coisimple.com domains
Independently confirming policy expirations
Mathurs LLC does not guarantee real-time alert delivery.
5. Service Availability & Interruption
The Services are provided on an “AS IS” and “AS AVAILABLE” basis.
We do not guarantee:
Continuous availability
Uninterrupted service
Error-free operation
Immediate processing
Service interruptions may occur due to:
Scheduled maintenance
Cybersecurity incidents
Infrastructure failures
Third-party outages
Force majeure events
Mathurs LLC shall not be liable for any damages resulting from service interruption.
6. Limitation of Liability
To the maximum extent permitted by law, Mathurs LLC, its officers, employees, contractors, and affiliates shall not be liable for any:

Business interruption losses
Insurance coverage gaps
Regulatory penalties
Contractual disputes

AI extraction errors
Missed alerts
Compliance status determinations
Data inaccuracies
Service interruptions
Aggregate Liability Cap
In no event shall Mathurs LLC’s total cumulative liability exceed the total subscription fees paid by you in the three (3) months preceding the event giving rise to the claim.
7. Indemnification
You agree to indemnify, defend, and hold harmless Mathurs LLC from any claims, liabilities, damages, costs, or legal fees (including third-party claims) arising from:
Your reliance on AI-extracted data
Your failure to verify extracted information
Your compliance decisions
Your breach of these Terms
8. Governing Law & Venue
These Terms shall be governed by the laws of the State of North Carolina.
Any dispute arising under these Terms shall be brought exclusively in the state or federal courts located in Wake County, North Carolina.
